This project took place in an 8-bed intensive care unit (ICU) at St. Bernard Parish Hospital, Chalmette, Louisiana, a small, 40-bed community hospital. Hospitalists provide medical service to patients in the ICU, with an intensivist on site 3 times a week and available tele-ICU capabilities.

Before participating in the Clinical Scene Investigator (CSI) Academy program, the unit had no structured inter-professional rounds, which had resulted in missed opportunities for nursing collaboration, advocacy, and patient and family involvement in care delivery. Because studies show adverse events are often due to lack of communication, our CSI team wanted to implement interprofessional bedside rounds.

We surveyed staff about communication related to the plan of care and rounding. On the basis of the survey results, the CSI team decided to implement a structured and collaborative rounding process.
